Abstract
The community based integrated care system has a new beginning in Korea. The paradigm of medical treatment and the
surrounding environment has been changed along with the increase of the elderly population. Since 2000, Japan has been
developing the community based integrated care system, which started at northern Europe in 1950’s. Recently, Korean government proclaimed the start of the “Community Care System”, which means community based integrated care system of Korea. It was concerned how we could make preparing for a good joining of dental care in “Community Care system”. Appropriate dental management protocols should be provided according to the functional level of the elderly in Community-Care. Approximately 10% to 15% of the elderly in community are reported to be in need of help in daily life or in a frail condition by KFI evaluation. This stage requires a strategic approach to dental care that does not exacerbate muscular and motor functions by providing care and rehabilitation of the oral function to the frail elderly so that nutritional management through proper meals is achieved. At the frail oral function stage, management of teeth and prostheses for masticatory function, oral hygiene improvement for enhancing oral ability, early evaluation of eating disorder/ dysphagia, and rehabilitation therapy of oral function should be provided comprehensively. For the about 10% of functionally dependent elderly, such as long-term care recipient who are in community or dementia patients, it is necessary to focus on the prevention of aspiration pneumonia by improving the oral hygiene along with rehabilitation treatment for oral function through early diagnosis of dysphagia.
