Treatment of Skeletal Class II Patient with Protrusive Lips and Gummy Smile Using an Antero-Posterior Lingual Retractor

Abstract


This case report describes the application of APLR for a patient who needed anterior torque control and intrusion of anterior teeth. After total 24 months of treatment, the superimposition between pretreatment and posttreatment showed slight retroclination of the maxillary incisiors (FH-U1 angle, from 114.0¡Æ to 112.0¡Æ), 4.5 mm intrusion and 5.0 mm retraction of the incisal edge. In spite of a large amount of retraction, the inclination of maxillary incisors was maintained well. The overall amount of intrusion of the maxillary molars was 2.0 mm, which was followed by autorotation of the mandible. The APLR system produced excellent and efficient retraction with good torque control and significant intrusion of the anterior segment. This system approach would be an effective option for skeletal Class II with the protrusive lips and gummy smile patient.
